why did my subwoofer catch on fire?

Had a shop install my jl audio dvc w3 in a 2010 lexus gs w the mark levinson system. The amp is a jl audio 5 channel. He ran the jl amp thru the factory inside speakers, not sure if he was still using the mark levinson amp or not, and used the fifth channel to run the sub. Before he put the sub in, he said one of the voice coils was blown. He said he can still put it in, but it wouldn't play as loud and will probably blow soon if u play it too loud. So I figured why not just throw it in for a while if it will still play. Two months later out of nowhere, and I wasn't even playing it loud, the sub caught fire. I smelled smoke coming from the trunk, opened it, and the sub was literally in flames. I had to pull the box out and throw it on the ground. Nothing else was damaged, but it coould have been really bad. My question is how could the sub have possibly caught fire? Was this an installer error? Was this an equipment error? Who's fault is this? The amp should still be ok, but not sure. Any help would be great
